Tập trung phòng, chống dịch cúm A (H5N1)
07:06' 09/04/2010 (GMT+7)

Vietnam: Strengthening flu A/H5N1

Thursday, 8/4/2010, 14:6 (GMT +7)

On 6 / 4, Department of Preventive Medicine and Environment (MOH) has confirmed cases positive for influenza A/H5N1 in Bac Kan province. He TVH patients., 22, Na Tao Village, commune such as grass, Cho Moi district, Kan Bac. It is cases of influenza A/H5N1 6 of Vietnam since early this year. So within three months of the year, the number of cases of influenza A/H5N1 was reported in our country has a higher total of all cases in 2009. SK & DS reporters had a quick exchange with other departments of the Ministry of Health functions prevention of influenza A/H5N1.

Eng. Level of patients being treated for H. influenza A/H5N1 infection in the Central Hospital of Tropical Diseases. Photo: Minh Tran

Eng. Intermediate Nguyen, Deputy Director of Emergency Department, Hospital Central Tropical Diseases: "Here the patient can live poultry death phenomenon."
Patient H. hospitalized in very serious condition, pale body, thought most organs, including lungs was most severe (severe respiratory failure), heart and kidneys also began to suffer.

According to epidemiological investigation, in which patients live with the phenomenon of dead poultry. As it is also at the commune there are 4 more people with symptoms of high fever and cough, are isolated, tracking. But the health of all four men are stable.

Eng. Alert level, people in areas with sick poultry phenomena, all die should be vigilant and to immediately notify authorities and agencies in the locality. In addition, the appearance of symptoms such as fever, cough, difficulty breathing, especially after contact with poultry you should think of influenza A/H5N1 and to right the nearest treatment facility for counseling, according Health monitoring and detection, timely treatment.

According ThS.Cap, now, a tropical disease hospital was put into use isolation rooms with negative $ 250,000 funding by Oxford University funding for isolating, treating patients with SARS, influenza A/H5N1 . However, if large numbers of patients, the hospital will have to mobilize more of the other preventive service for the treatment of patients with dangerous infectious diseases.

TS. Luong Ngoc Khue-director of the Department of Health, Ministry of Health: "The hospital is always ready to take the initiative in treatment services."

The weather this season easily create favorable conditions for virus spread and development in poultry as well as the risk of spreading avian influenza and can spread easily to humans.

Therefore, the Department of Health Management has requested a written emergency hospitals, hospital beds under the Ministry, the Provincial Health Department, and city health departments plan to urgently when there is active outbreak out; a permanent plan, the reserve of drugs, fluids, equipment, materials and layout of the number of hospital beds, emergency facilities available to ensure the content of the collection, treatment and isolation patients according to guidelines for prevention.

Related to the treatment of influenza A/H5N1 epidemic, he said Khue, Health Administration has asked the hospital last line in the treatment system must be ready to receive the flu treatment of severe cases and difficult to treat, and support facilities on the lower level of health prevention services.

Accordingly, the receive and treat patients and the average online by the hospital treating influenza primarily in charge of the system. Private line extensions and treatment system in place in the treatment of influenza, will take on the detection of suspected cases in the community to guide and coordinate, online social support in surveillance, treatment and fight against epidemic.

TS. Nguyen Van Binh, deputy director of the Department of Preventive Medicine and Environment: "Do not be" sleeping "on the service."
Currently avian appear scattered in several provinces. Besides, cold weather, humidity, the virus will have the opportunity to develop and spread in poultry should spread the risk is great. The people should cooperate with authorities and agencies in detecting the phenomenon of bird illness, death and especially not in use, slaughter, transport, trade poultry of unknown origin, especially in the area flu outbreak in poultry. Advocacy on issues of mass media for people to understand and comply with the Health Ministry's recommendations on measures against bird flu should be "temperamental predominance" again. Because the service via a long lull, it seems that people and even the authorities in some localities as "sleeping" makes the disease a chance to come back and threaten public health .

Vietnam: Global Overview-January 2010-Prepared by EMPRES/FAO GLEWS

Excerpts:

The question still remains as to where the virus hides during inter-epidemic periods in the complex and variable poultry production systems comprising chickens, ducks and rice fields. Consistent outbreak investigations are not undertaken on infected farms and key information is usually missing from the field.

In January 2010, a human case was observed in a 3-year old female from Khanh Hoa Province. The source of exposure is under investigation. Her family raises chickens but did not report any mass poultry illness or death.

Based on the monitoring of surveillance activities, three currently circulating virus clades have been isolated: (1) HA Clade 1 (predominant in southern Viet Nam and also isolated in Cambodia); (2) HA Clade 2.3.4 (predominant in northern Viet Nam and also circulating in China); and (3) HA Clade 7 (detected in poultry seized at the Chinese border and at markets near Hanoi). In 2009, ten viruses were sent to the U.S. Centers for Disease Control and Prevention (CDC) for sequencing and, to date, no new circulating clade has been detected.
A recent study by Carrel et al. (2010) showed that there have been at least six independent H5N1 introductions into Viet Nam and there were nine newly emerged reassortants from 2001 to 2007. H5N1 viruses in Viet Nam cluster distinctly around Hanoi and Ho Chi Minh City. More information is available at…

From: http://www.plosone.org/article/info%3Adoi%2F10.1371%2Fjournal.pone.0008631:
While the interactions between time, space and genetic evolution could potentially be circumvented by repeated introductions of H5N1 strains of different lineages into Vietnam, we would still expect to see the northern region around Hanoi and the southern region around Ho Chi Minh City acting as important sites of genetic differentiation rather than the central region of the country.

Genetic distance between case pairs is higher in 2005 and 2007 than in the first two years of the dataset, an indication that these influenza viruses have evolved as local epidemic strains in domestic poultry after sweeping over Vietnam during the 2003–2004 outbreaks.

By 2007, however, genetic distances are similar for the within- and between-region categories. The significant three-way interaction (Table 1) indicates that genetic variation in Vietnamese HK821-like cases is systematically different between regions, years and gene segments, and that these three factors interact in ways that affect genetic distances between virus case pairs.

Excerpts:
According to the last surveillance report, between 28 December 2009 and 7 February 2010, 17 419 active surveillance samples were received at the High Security Animal Disease Laboratory (HSADL), Bhopal. Testing was completed on 14 083 samples (some from the previous month), all but 20 (all from West Bengal) with negative
results, and another 12 180 were under test or pending.

Egypt: Global Overview-January 2010-Prepared by EMPRES/FAO GLEWS

Confirmed outbreaks of H5N1 HPAI in Africa (Egypt) over the
last six months are presented in Figure 5.

Excerpts:
Compared to 2009, when most cases were in children under four years of age, five out of the seven human infections so far in 2010 have been in adults. The observed CFR is also higher so far in 2010 (43% compared to 10% in 2009, although it had decreased from 50% in 2008). The relative increase in H5N1 HPAI outbreaks in poultry (50 in January 2010 compared to seven in the same month in 2009) is a result of the improved surveillance and reporting rate, mainly because of the effectiveness of CAHO.

During January 2010, 116 farms in five governorates were subjected to active surveillance and no HPAI infections were detected. On the other hand, two of the nine HPAI notifications (passive surveillance) received from commercial poultry farms were found positive for H5 HPAI.

Global Overview-January 2010-Prepared by EMPRES/FAO GLEWS

Issue No. 19

Excerpt:

Data from previous years have shown a peak in the number of outbreaks/cases during the January-March period in both poultry outbreaks (Figure 11) and human cases (Figure 4), although there is a decreasing trend in the height of the peak as years go by.

A similar, although not so evident trend, is observed when looking at the number of countries reporting outbreaks (Figure 10). As expected, during January 2010 we
have seen an increase in the total number of outbreaks and countries affected. January constitutes the peak so far (both in terms of number of outbreaks and number of affected countries) since the previous January-March period.

Against the decreasing trend observed since 2004, it seems that the peak height in January 2010 is very similar in magnitude to that of 2009.

Moreover, in terms of number of outbreaks, 2010 has already surpassed the peak of 2009, which was also reached one month later (February 2009).

This may be partially explained by the implementation of more intensive surveillance programmes in countries such as CAHO in Egypt, together with the fact that vaccination of backyard poultry was stopped in July 2009). It may also be related with a reduction on the efficacy of control programmes in endemic countries (fatigue).

During January 2010, H5N1 HPAI has re-surfaced in several countries, where the disease was believed to have been eliminated (without vaccination): Cambodia, Nepal and Israel.

It is still too early to evaluate whether these are reintroductions, where wild birds and hunting/poaching play a significant role, or whether the virus was circulating
undetected by surveillance programmes.

In the cases of Bangladesh and India, that had reported no outbreaks in the
second half of the year, a new wave of cases has been observed since the beginning of 2010.

H1N1 Outbreaks in US, Abroad Carefully Monitored

H1N1 Outbreaks in US, Abroad Carefully Monitored PDF Print E-mail

'We are not seeing any significant evidence of a resurgent ‘third wave’

As the deadly H1N1 flu is making a comeback in places around the world, including in the southwest United States – Georgia in particular, which has been hit particularly hard – authorities are wondering whether these new outbreaks of the influenza virus represent a worrisome “third wave,” a new mutation, or whether something else is in play that’s responsible.

Alabama and North Carolina also have reported outbreaks, Centers for Disease Control and Prevention (CDC) officials reported in their latest update on the spread of the virus. CDC warned Georgians to get vaccinated for the H1N1 flu. Georgia has one of the lowest rates of vaccination among its population, which some experts believe is likely contributing to the increase in new infections there.

Local activity has also been reported in Arkansas, Louisiana, Mississippi, Tennessee, Virginia, Hawaii, New Mexico and Puerto Rico.

Across the world in Malaysia, new A H1N1 clusters have been detected throughout the country, putting the country’s Health Ministry on high alert.

Between March 21 and 27, 83 people in Selangor, Malacca and Terengganu were confirmed infected with the virus.

Health Director General Tan Sri Dr Ismail Merican said the sudden clusters indicate the H1N1 virus is still active, but stressed that the government is on top of the new outbreaks.

The World Health Organization (WHO) reported that the H1N1 pandemic influenza remains the most prominent influenza virus circulating around the world and that the virus remains active in parts of the tropical zones of Asia, the Americas, and Africa.

Elsewhere, "an increasing trend in pandemic influenza activity for at least one week during March," according to the WHO, occurred in Guatemala, Nicaragua, El Salvador, Panama, Brazil, Peru, and Bolivia all reported.

“At present, we are not seeing any significant evidence of a resurgent ‘third wave’ in the US,” the “notable exception [being] the recent sharp increase in cases in Georgia,” HSToday.us was told be a senior government public health preparedness official.

“CDC was concerned enough to hold a conference call last week, where we learned that the state had this absolute lowest vaccination rate in the country last year. Thus it was left with a vulnerable population at a time when the virus was still in circulation.

The official said “the average vaccination rate for the whole country stands at ~ 23 percent.”

In Connecticut, in comparison, the rate is one of the highest in the country, at 43 percent.

“Other New England states also fared well in this regard, a result of their intense regional cooperative planning,” the official noted.

“Regarding H5N1, there have been sporadic reports of minor changes in viral structure, but none that have thus far led to increased transmissibility,” the official added, saying that “virulence remains the same with a consistent 60 percent mortality rate in the human population. The most recent cases are still coming from Egypt and Viet Nam. Overall, WHO’s case count now stands at 492 documented cases, with 291 associated deaths.”

Authorities continue to be concerned that as pandemic H1N1 comingles with the highly fatal H5N1 bird flu there could be a genetic “reassortment” of the two viruses, potentially resulting in a deadly new flu virus.

Writing in his latest bulletin, Michael Osterholm, director of the Center for Infectious Disease Research & Policy (CIDRAP) and director of the National Institutes of Health-supported Center of Excellence for Influenza Research and Surveillance within CIDRAP, said “recent novel H1N1 influenza developments in the southeastern region of the United States merit … attention. They're prime examples of why we must vigilantly expect the unexpected.

Osterholm pointed out “an unusual level of hospitalizations linked to lab-confirmed novel H1N1 influenza in the state of Georgia prompted CDC to call a short-notice press briefing and describe the uptick as a ‘worrisome trend.’”

“Most of the hospitalized patients are adults who have chronic medical conditions and did not get the H1N1 vaccine,” Osterholm wrote, adding “Georgia had one of the lowest rates of H1N1 vaccination last fall, and people with chronic medical conditions are among the groups considered at high risk of developing complications if they're infected by the novel H1N1 pandemic virus.

“I don't know the last time in April we've seen an increasing number of hospitalizations for influenza,” Osterholm stated, adding, “of note, the number in Georgia is similar to the number of hospitalizations seen at the height of the second pandemic wave last October. In short, the trend is unexpected. Does it portend a third wave of the pandemic? We just don't know … But I sure wouldn't rule out the possibility.”

“For now, what we do know is that what's happening in Georgia is unusual for this time of the year and that people who are getting sick enough to need hospital care did not get vaccinated and are among the groups considered to be at high risk of developing complications,” Osterholm wrote.

Meanwhile, researchers have reported in a new a study published in Science Translational Medicine that the H1N1 swine influenza virus shares an important surface protein with the virus responsible for the 1918 flu.

Swine flu scare was unnecessarily exaggerated: WHO

STAFF WRITER 17:30 HRS IST New Delhi, Apr 6 (PTI) A top official of the World Health Organization today admitted that the global swine flu scare was unnecessarily exaggerated, an allegation the international body faced when the killer disease claimed thousands of lives in several countries including India.

"Yes, yes," said Dr Samlee Pilanbangchang, regional director, South-East Asia, WHO, when asked whether the swine flu scare was unnecessarily exaggerated.

He was addressing an function here on the eve of World Health Day.

When the WHO official made the candid admission his colleagues in the organisation intervened and he did not speak further on the matter.

WHO was criticised last year for its handling of the swine flu pandemic and was accused of exaggerating the danger of the virus under pressure from different countries and drug manufacturers.
